Choosing between two OMS offers

I’m weighing a hospital-employed role with q3 call and CRNA support against a two-op private practice running IV sedation only; after 12 years, I won’t compromise on crisp flap design, dependable instrument turnover, and a tight multimodal pain plan (long-acting blocks plus scheduled NSAIDs and acetaminophen). How have you traded compensation for OR access, skilled assistants, and PACU coverage?

Quick win: a Trellis judge‑analytics CLE showed us to save “Judge + motion type” searches with alerts and drop a one‑page rulings snapshot into the intake packet, which cut our rule‑check time; caveat, coverage is uneven outside their states. https://trellis.law/webinars Are you mostly state or fed?

Leaning hospital if they’ll put it in writing: guaranteed half‑day OR block and a named 1:1 assistant; I took a lower base for that and it paid off when trauma/ASA III stacked up. I also baked in “two third‑molar sets + 24‑hour turnover” and pharmacy stocking so my “long‑acting blocks + NSAIDs/acetaminophen” wasn’t a debate. The two‑op IV setup is fine for volume, but without OR access your complex cases bottleneck fast.

Piggybacking on @sophie_w93, we kept Mindbody and set up a Zapier→Twilio text that fires 12 minutes before 6:00 a.m., merges first name/time, and adds a random tip from a Google Sheet (“bring water!”-short) — a pre-class nudge and sip-of-water in one ping. Caveat: it’s two tools, but cheaper than upgrading; if you want one system, Vagaro’s SMS templates can handle this too. Happy to share the Zap steps if you want.
